Procedure description: Authorization of humanitarian entry in favor of Cuban citizens who have emigrated to visit their close relatives who have died or are seriously ill.

Requirements:

• Updated identity card of the person who serves as reference in Cuba.
• Summary of the patient's medical history in Cuba or death certificate, as appropriate.
• Application form for entry to Cuba (AP-1).
• Reference person form (AP-3).
