Documents to be presented:
Identity card of the person.
Food products notebook
Affidavit of Medical Diet, signed and minted by the Public Health Care Center that issues it and they must state the length of pregnancy that the pregnant woman has 26 weeks or more.

Documents to be submitted by consumers in case of:

Loss of the food products notebook

A member of the nuclear family over 16 years old with an identity card.
Stamp of $10.00.

Substitution by deterioration of the food products notebook.

To process a diet must be presented:
- Food Product Notebook
- Form No.12 Affidavit of Medical Diet, duly signed and minted by the Public Health Care Center that issues it.
- The renewal of the medical diet may be processed by the consumer on any day of the month in which the current diet expires.
